What is an Open Rhinoplasty?

Open rhinoplasty is a surgical approach that allows for the reshaping of the nose by creating a small external incision across the columella, the narrow strip of skin between the nostrils. This technique allows Dr. Paquet to lift the nasal skin and gain full access to the underlying cartilage and bone, making it easier to perform complex or precision-based changes. Dr. Christian Paquet performs open rhinoplasty at his Phoenix, Arizona, practice to help patients achieve refined results that improve nasal function and facial harmony.

Am I a Candidate for Open Rhinoplasty?

This method is often recommended when significant structural adjustments are needed, such as modifying the nasal tip, correcting asymmetry, or repairing nasal trauma. Because of the increased visibility and control it provides, open rhinoplasty is often the preferred choice for more advanced or revision cases. Patients who benefit most from open rhinoplasty typically want to correct prominent features such as a drooping nasal tip, a crooked nose, or breathing issues caused by structural defects. The procedure is appropriate for healthy adults who are non-smokers and have realistic expectations about the procedure results. It is also essential that facial growth is complete, which is generally by age sixteen or older. Dr. Paquet will perform a full nasal examination during your consultation, review your health history, and discuss your goals. Open rhinoplasty may be the most appropriate option if your desired changes are complex, if you have had previous nasal surgery, or if there is a significant functional concern, such as a deviated septum.

Benefits of Open Rhinoplasty

One key benefit of open rhinoplasty is its accessibility to the internal structures of the nose. By lifting the nasal skin, Dr. Paquet can visualize and manipulate cartilage and bone with greater precision than is possible through closed techniques. This helps reshape the nasal tip, correct asymmetries, or addressing issues from prior surgeries. While there is a small external scar, it typically heals well and becomes barely noticeable over time.

Types of Open Rhinoplasty

Open rhinoplasty can address a wide range of concerns, and each procedure is customized based on whether the goal is cosmetic, functional, or both.

Cosmetic Open Rhinoplasty

Cosmetic open rhinoplasty, or aesthetic rhinoplasty, enhances your nasal appearance to better fit the overall facial structure. The cosmetic surgery might include smoothing a dorsal hump, addressing nasal cartilage, straightening nasal bones, refining the tip, or narrowing the nostrils.

Functional Open Rhinoplasty

Functional open rhinoplasty corrects breathing problems caused by structural abnormalities such as a deviated septum or collapsed nasal valves. Many patients who have experienced chronic congestion, snoring, or sinus infections find significant relief after this surgical procedure.

Revision Open Rhinoplasty

Revision open rhinoplasty is used when patients are unsatisfied with the results of a previous nose job surgery or have experienced complications. Because revising previously altered tissue is complex, the open surgical technique approach gives Dr. Paquet the control needed to make precise improvements to improve your facial appearance and overall facial balance.

REAL PATIENT REVIEW

“I am so grateful that Dr. Paquet was up for the challenge of fixing my nose. I had surgery about 25 years ago with a doctor who destroyed my nose both inside and out. It was crooked and I used to have terrible panic attacks because I couldn’t breathe. Other doctors I saw didn’t want to even attempt to fix it. Luckily one doctor did refer me to Dr. Paquet and he said he would do the surgery. I am happy to say that I no longer have panic attacks and my nasal passages are wide open now. As a bonus my nose is not crooked anymore either!!!! I highly recommend Dr. Paquet not only for the wonderful work he does, but because he cares. He takes the time to listen to your concerns and will explain everything to you in detail. I was so paranoid about having nose surgery again after the horror of the first one, but he helped me get through it. I’m so glad I did, it has given me a much better quality of life. Thank you a million times over.”

Before Your Open Rhinoplasty Procedure

In preparation for your open rhinoplasty surgery, it is essential to follow these steps to ensure optimal results and minimize any risks. If you smoke, you will need to quit several weeks in advance, as smoking can interfere with healing. You will also be asked to stop taking blood-thinning medications, supplements, and alcohol in the weeks leading up to surgery, as these can increase the risk of bleeding and bruising. You should plan to arrange for a caregiver to take you to and from the surgical center and help you during the first day or two of recovery after your nose surgery. Dr. Paquet’s team will provide detailed instructions about what to expect and how to prepare, including picking up prescriptions and setting up a comfortable recovery space at home. On the day of your plastic surgery, you should arrive with a clean face and avoid wearing any lotions, makeup, or jewelry.

The Open Rhinoplasty Procedure

Dr. Paquet typically performs open rhinoplasty under general anesthesia, which allows for the safest and most controlled environment. Most procedures take two to three hours, although the exact timeline depends on the complexity of the surgical plan.

The Incision

The procedure begins with a small, precise incision made across the columella. Once the incision is made, Dr. Paquet carefully lifts the nasal skin to expose the underlying cartilage and bone fully. This step allows for such a high degree of control and accuracy during the reshaping process.

Reshaping the Nasal Structure

With the internal structure exposed, Dr. Paquet can make the necessary changes to the cartilage and bone. This may involve smoothing a bump on the bridge, narrowing the nasal tip, straightening the septum, or reinforcing weakened areas using cartilage grafts. Every adjustment is made with careful attention to aesthetics and function, ensuring that the nose looks better and works better.

Closing the Incision

Once the desired changes have been completed, the nasal skin is re-draped over the new shape and sutured back into place. Although a small scar is left from the external incision, it is discreetly located and typically heals to a thin, faint line. A splint is usually applied to support the new structure and minimize swelling. Soft packing may sometimes be placed inside the nostrils to help maintain alignment and support the healing process.

After Your Open Rhinoplasty Procedure

After rhinoplasty surgery, most patients can return home the same day. In the days following the facial plastic surgery procedure, it is normal to experience swelling, bruising, and mild discomfort. Dr. Paquet will prescribe medication to help manage pain and provide instructions on how to care for the incision and manage post-operative symptoms. You will notice the most swelling and bruising in the first week, particularly around the eyes and cheeks. Most patients feel ready to return to work or light activities within seven to ten days. You should avoid strenuous exercise, bending, or lifting heavy objects for several weeks. Although much of the swelling resolves within the first six to eight weeks, residual swelling can take up to a year to subside entirely. Dr. Paquet will schedule regular follow-up appointments to ensure your nose is healing as expected.
